Forms
The Office of the University Registrar strictly follows University Guidelines for the Honor Code. Please be aware of this when filling out any of the forms listed below.
Please fill out the appropriate form, obtain all required signatures, and return the form to the Office of the University Registrar in Student Union I, Room 2101. If any forms are due to the Office of the University Registrar on any of the dates listed on the academic calendar, they must be submitted by 5:00 p.m. on the deadline. Please have a valid photo I.D. when picking up and dropping off forms in person. Electronic photos or copies of a photo I.D. will not be accepted.
All forms submitted are normally processed in two to three business days, not including the date received. There are several peak periods, (graduation, the priority registration period etc.) when processing time may take longer.

All Students
Form | Description |

Withdraw Graduation Application | For use when withdrawing your graduation application for the current term. |
Veterans Registration Reporting (VRRF) | The University certifies enrollment to the VA based on the information provided on the reporting form. |
Verification of Pending Degree | |
Updating Race/Ethnicity | For Students (Faculty/Staff, please work with H.R.) to request a change to your ethnicity or race |
Time Conflict | For students requesting to register for a course that results in a time conflict of 15 minutes or less |
Substitution/Waiver Form | Request that a requirement in an academic program be met by: 1) a transfer course even though not considered equivalent to a Mason course, or 2) a Mason course not usually applied to meet the requirement. Also to request that, on some clearly-detailed basis, a requirement in the student's academic program be waived. (Does not waive or give credit hours.) |
Study Elsewhere Removal | Submit to the Office of the University Registrar to remove a previously requested Study Elsewhere. |
Study Elsewhere - Undergraduate Students - Graduate Students | Must be submitted to the Office of the University Registrar prior to the taking of the course at another institution. |
Special Registration to Retain Active Status (ZREG 200) | For students not enrolled in a credit-bearing course but whose academic department certifies that they are pursuing an activity related to their Mason enrolled program can retain active status by registering for Special Registration (ZREG 200). |
Solomon Amendment | Requests for student recruiting information must be made in writing, using military letterhead which clearly identifies the military recruiting branch or office. |
Setting Up Your Patriot Pass Account | |
Senior Citizen Tuition Waiver | Senior citizens must first be admitted through the Non-degree Office before they may register. For more detailed information go to the Senior Citizen Tuition Waiver Program page. |
Satisfactory/No Credit | Must be submitted to the Office of the University Registrar by the last day to add. |
Retroactive Graduation Application | |
Request to Prevent Disclosure of Student Information | Used to place a hold on student accounts to prevent student information from being released, or to revoke a hold currently on a student’s account. |
Preferred Name | To modify a student's first name |
Late Graduation Application | Used to apply for graduation after the online application is no longer available. A Retroactive Graduation Request is required after the 6 week grace period has passed. |
Incomplete Grade Contract | For use when an advanced deadline will apply; when incomplete work will be reviewed by another faculty member, or in other unusual circumstances where special clarity is needed. |
Incomplete Extension | Petition for extending the deadline for submission of completed work. |
Graduation Invitation Fall 2019 | For use by international students. Submit as an addendum to enrollment verifications available from Patriot Web or degree verifications available from the National Student Clearinghouse. |
FERPA Consent to Release Student Information - online form | For students to grant the University permission to release FERPA protected student information to a third party |
Enrollment Verification | Verification of enrollment. |
Document Authentication Request | For authentication of university academic documents. |
Diploma Accent Mark Request | For students to request adding an accent mark to your name as printed on your diploma. |
Degree and Enrollment Information | Submit to third parties as an attachment to Clearinghouse verifications as needed. |
Credit Overload | Used to request a credit overload. Can be used by Graduate, Undergraduate and Non-Degree students. |
Course Audit | Must be submitted to the Office of the University Registrar by the last day to drop. |

Change of Personal Information | Used to request a change or update to name, birth date or social security number. |
Change of Calendar | To be completed by students who want to change calendars in which their program is offered |
Certificate of Tax Dependency | For students to grant the University permission to release FERPA protected student information to a parent or legal guardian if the student is a dependent. |
Before Leaving Mason | To be used as a checklist if you leave George Mason University. |
Undergraduate Students
Form | Description |

Student Success Plan | Students requesting permission to repeat a non-repeatable course beyond the allowable limit use this form to document a plan for completing a course successfully. |
Selective Withdrawal | For Undergraduate, Degree seeking students. |
Secondary Degree Declaration | For undergraduates who wish to pursue two concurrent degrees |
Reserve Graduate Credit | Must be submitted to the Office of the University Registrar by the last day to add. |
Reassignment of Non-Degree Credit (Undergraduate) | For current Undergraduate Degree seeking students to transfer previous Mason Non-Degree work into their current Undergraduate program. |
Re-enrollment Application - Undergraduate | |
Minor Declaration | Used by currently enrolled bachelor's degree students who would like to declare a minor. |
Leave of Absence | Required for all undergraduate students who are planning an absence from Mason. |
Graduate Course for Undergraduate Credit | Must be submitted to the Office of the University Registrar by the last day to add. |
Delete Program Request Form (Undergraduate) | For currently enrolled bachelor's degree students who would like to delete a program (ex. major, minor, concentration). |
Change/Declaration of Academic Program | For undergraduate students who wish to declare or make a change to their Academic Program |
Certificate Declaration | Used by currently enrolled bachelor's degree students who would like to declare an undergraduate certificate. |
Bachelors Accelerated Master's Transition | Used with the bachelor's graduation application; Only for those accepted into an accelerated master's program. |
Academic Advisor Approval | Must be submitted to your Academic Dean before you may register for classes after returning from suspension. |
Graduate Students

Waiver of Residency (Graduate) | For graduate students to request an exception of the residency requirement |
Voluntary Resignation from Graduate Academic Program | Students who have been granted a resignation will not be able to register for any courses unless admitted to another degree program or non-degree status in a different program. |
Transfer of Credit (Graduate) | For Graduate students to transfer credit from previous institutions attended or from Mason Non-Degree. |
Time Limit Extensions | For students who need to extend their graduation deadline or advancement to candidacy deadline. |
Termination Appeal (Non-Degree Graduate) | Submit to your College upon notification of possible Termination. |

Reduction of Credits | For Doctoral, MFA or Masters Degree students in programs of more than 39 credits |
Re-enrollment Application - Graduate | To be completed by Graduate students who have missed two or more consecutive semesters at Mason. |
Master's Thesis Research | Grace Period for Full-Time Equivalent Status. |
Master's Degree Non-Course Requirements | Used to report the completion of all Masters Non-Course Requirements. |
Graduate Student Request for Undergraduate Course Registration | |
Graduate Program Resignation and Transfer | For students who wish to resign from one graduate program and transfer to a lesser degree program or certificate in the same discipline |
Dual Masters Program of Study | For students who wish to pursue two master’s degrees simultaneously. It must be submitted upon matriculation into the second program. |
Dual Degree Request for Program Deletion (Graduate) | |
Doctoral Dissertation Research | Grace Period for Full-Time Equivalent Status. |
Doctoral Advancement to Candidacy and Non-course Requirements | Used for Doctoral students to be advanced to candidacy |
Change of Program (Graduate) | This form can be used if the program change is within a single college. College changes require re-admission. This form cannot be used by newly admitted students. |
Graduate Termination Exception Request | Submit this form to your College upon notification of possible Termination. |
Non-Degree, Mason Korea, OCPE Students
